Well the Kage is like the head of the military but instead of just commanding they go into battle as well, so if things get rough they have to fight the hardest of battles, meaning they have the highest chance of dying. But we also don't really know how a lot of Kages died.

well for one when the kage signs up to be kage they basically do know the basis of being a kage which is to be the champion of the village and to defend the village with their life.

Well the Third should have died of old age, and he kinda did basically...
So it not a must... i mean it is basically what any ninja should be prepared to do, except that the Kage is responsible for the whole village and is willing to give his life for the greater good.

Think of it this way, no one who wouldn't sacrifice their life for the village would even want to be a Kage in the first place. They make it their dream to become Kage because they love their village and want to be it's protector.

It's just that when it all comes down to it, they're willing to do what they have to to protect their home. Even give up their life. That's all

Thing is, is that for the life of a ninja, which is the life of a warrior, dieing by natural causes can be seen as cowardly. Warriors would prefer to die in battle, as it's the "honorable" way to go. I imagine it'd be the same mentality.

Also keep in mind, that the nations are also quite young. There's been little to no chance for legitimate stability. It's still a very tense warring world they live in.
