Which made me think -- whenever Itachi brings out Totsuka and Yata he is portrayed as being invincible...

This is probably the reason why Itachi did not bring out Totsuka/Yata against Kabuto. If he did, Kishimoto would have been obligated to have Itachi immediately overwhelm Kabuto (which is anti-climatic story writing).

The only weakness to IM Itachi would be that he can't maintain it forever. Itachi stops being invincible once he doesn't have Totsuka/Yata out.

Itachi is untouchable while Yata mirror, Susano'o, Sharingan, and Totsuka blade are active. However, he has a time limit on them which hampered his ability to use them. And of course plot aids brought him back down to human levels as well.

All the strongest characters with the most potential die of disease because they are untouchable by an external threat.

Not true. They are tools, the same as the sage's (albeit more powerful as the sage's tools are not ethereal spirit weapons.) They are not ninjutsu and they are not made of chakra.

The part about kabuuto is a bit true. It would have simply been very unthrilling to watch kabuto fight 2 Complete susano'o; 1 having Yata mirror which would deflect all his attacks. That's why kishi just made the uchiha camp ribcage.

Honestly ask yourself, would kabuto's fight be fun if it was just susano'o vs whatever OP kabuto would pull out?

It could, but ask yourself if any jutsu to get under susano'o would work on itachi, even without susano'o? Kakashi attempted to attack from below and his clone was caught without much of a fuss. Just because a jutsu can get around or under it, doesn't mean it will.

True, but that's mostly because Sasuke wasn't allowed to kill Kabuto and his techniques are usually meant for killing people in one hit. To be fair, Itachi didn't have killing intention either. The only reason I believe DSM Kabuto beats MS Itachi is mostly because Itachi would over-use his MS before he manages to finish off Kabuto.​
