I could see with Perfect Susanoo since the criteria was never known but Susanoo requirements was known through Sasuke and Itachi met them in part 1. Kakashi situation is special since ALTHOUGH he had MS since part 1 he seemed to not have the stamina to use it.

It was ONLY till Itachi mentioned requirments for susano'o and then brought it out is when we knew about it=power-up.

Kishi could have written anything he wanted, since susano'o wasn't mentioned before that event its a power-up whats so hard to grasp?

I think people seem to forget, that the Totsuka is not the blade of piercing. That's Orochimaru's Kusanagi, the Totsuka is for sealing IF it can pierce it's target. Totsuka isn't piercing Susano'o.

Exactly. It needs to Pierces Susano'o in order to seal it which it cant as sasuke's and madara's Susano'o are more powerful than Itachi's. Obito phases out for 5 minutes for the allotted 3 minutes itachi has to keep susano'o up and dies.
