You're assuming that the graduation exam was held once a year. If it was then Naruto failing 3 times is a plothole.

This was just horrible writing and should be referred to as a "Branch" in the story that Kishimoto wrote so he could reference it later if he wanted. This ended up poorly as he never explained the circumstances around it and it contradicts what we do know. Its more likely that Naruto was able to take the exam several times out of goodwill, a way to make him calm down and study. Like a kid who thinks he is so far above everyone else and refuses to do "normal" work. Allowing him to take the test and fail it would put him in his place.
