if you read the manga then you would know Tsunade was the 5th hokage was only picked as Kakashi did not want the role.

Tsuande became Hokage because Jiraiya did not want it. Hiruzen became Hokage again (after stepping down for Minato), when the 4th died.

I think depending on when Naruto is killed by Kawaki and what events follow. If it's not good(instability) prior to Narutos death then in such a crisis the village will need stable leadership and Kakashi will become Hokage again.
