If Itachi was Naruto's rival .

I'd say he'd be just the same. Sasuke is fitted to follow his plot line and be a good rival for Naruto. He was conceived primarily for the latter. But Itachi's personality might make it more interesting. Sasuke's cold shoulder gets redundant after a while.

I don't think it would work. Sasuke is the perfect yin foil to Naruto who is yang. Itachi just couldn't work as a foil to Naruto. He wouldn't be Itachi.
