EMS Madara? Maybe... Since he would be hellbent on being more powerfull he would take more care of himself, finding a way of combating his illness (maybe by forcing Tsunade or Kabuto to treat him), he would easily get Sasuke's eyes, with his charisma he could get people to follow him (guys like Kisame and Kabuto for example)
He would definitly be a force to be reckoned.

Itachi wouldn't have awakened EMS, because Sasuke didn't have MS until after his death, not during the fight. He'll just have 3 Tomoe again. Itachi would still suffered and died from his illness. Madara would've had a ball with Itachi.

coul we alll just tell the truth,itachi might be a genius or prodigy which ever you pick,but madara is the ultimate ninja who surpasses in every category,it was much better for itachi to have gone because madara would have soloed him back to death eg. Minato,harishima,tobirama,whole shinobi alliance,kages and to a point naruto and sasuke.

p.s:If sasuke knew his brother was so powerful and smart why didn't he ask him to stay fight while he ressuracted the previuos kages
