Between the Kyuubi Attack and the Uchiha Clan Massacre, a lot happened. Due to the Sharingan's ability to control the Kyuubi, the Konoha Elders believed that the Uchiha Clan was the mastermind of the attack. There were two ways to react to the situation and these were greatly illustrated.
Danzo and the Konoha Elders -They had very militant and archaic ideology. They believed if there is a threat it should be immediately removed and chances shouldn't be taken. These were shown through there wish to eliminate the Uchiha Clan. They also showed this when they wanted to destroy the new found Akatsuki.
Hiruzen, the Third Hokage - Hiruzen was almost the opposite of his rivals. The Third Hokage was a very peaceful man who wanted to end such situations with diplomacy and make a threat a benefit. Peace talks were happening until Danzo manipulated Itachi to kill the Uchiha.
But imagine, What if the Third Hokage retired from his position and named Itachi his successor?
What would be different?
Well for one Sasuke wouldn't be the corrupt and lost individual he is now: he probably would have still been a very powerful ninja. Naruto would still be the host of Kyuubi but I think with Sasuke not being so cocky, Naruto and him would have become friends (as Kushina wanted). Due to this and Itachi's love for his little brother, Naruto would have been treated better by the Hokage and in turn the village.
How would the 'history' of Konoha have changed?
What happened since the events of the Uchiha Clan Massacre is a better question. Well first the Sunagakure-Otagakure Joint Invasion of Konoha. Would this have happened? I, and many others are under the belief that this was a plan from Danzo to remove the Third Hokage and Fourth Kazekage but this could be false, but either way, whether it was Danzo or Orochimaru, the Third Hokage wanted to be removed and killed.
We know that Hiruzen is strong - arguable the strongest Hokage ever but at the old age of 69, Hiruzen was becoming a little rusty. Still strong, but not as strong. Itachi on the other hand, was probably in his prime at the time or just passed it. Itachi was feared by almost all - the man who could slaughter arguably the most powerful clan single handedly, the greatest Genjutsu ninja of all time, having Ninjutsu and Taijutsu that could strike fear into the strongest of ninja, Itachi was no doubt on of the strongest.
We know that Itachi 'dealt' with Orochimaru pretty easily with his genjutsu - Itachi could have sent Orochimaru crawling back to his reptilian friends if Itachi got real serious. Judging from that, Itachi could have stopped the Invasion, with the villages help of course.
Now afterwards, this is where the problem would begin : the Invasion of Pain. The whole time-skip thing wouldn't have happened so Naruto wouldn''t be the OP Sage Mode brotha we learned to love. He would be powerful - maybe Kakashi level at best but no better. Sasuke as well but a bit stronger.
So assuming this, Naruto would be kept hidden, with only Itachi and very few others knowing his location. The whole Uchiha force, the Shinobi and more might have been able to repel the attack a bit better, but assuming the time skip didn't happen, Tsunade and her awesome medic skills would help out so this could have went either way.
Itachi would put up a great fight - Assuming Jiraiya = Itachi, Itachi could take out at least 3 Peins and with possible knowledge, the whole attack could have been repelled. Konoha would have been heavily destroyed with a lot of fatalities.
The Fifth Kage Summit probably would have still happened but Sasuke wouldn't have crashed the party. So the Fourth Shinobi World War might have happened. According the Varrah's theory which I agree with, Itachi got sick because of his use of the Mangekyo Sharingan and jazz like that, but as Hokage he probably wouldn't have fought as much as he did while in the Akatsuki and would have stayed healthy.
The current battle would be a lot different and I'll let you guys deal with that
So what do you think? Do you think had Itachi been Hokage, Konoha would be in better shape now? I think so...
Oh and what would Danzo be doing? Itachi would have had him and the Konoha Council executed for Crimes against Humanity, Ethinic Cleansing, and Genocide!
