Here's my guess as to what happens in 678:
Ichigo explodes in a massive thingy of Reiatsu.
Orihime says "Kurosaki-Kun"
Renji and Rukia are heading towards the palace, and sense his power. They are surprised and think he has a chance (He doesn't)
Byakuya and Toshiro notice Ichigo's reiatsu, and think the same thing, being equally wrong
Uryu is slightly more bleeding out than last time, notices Ichigo's reiatsu, mentions it to Haschwalth. Hashwalth says nothing.
Yhwach says fighting him is pointless because blah blah blah blah blah.
Several full page spreads of dust settling.
Ichigo's new Bankai is shown behind lots of smoke that makes it impossible to make out any distinguishing features beyond the shape of the hilt.
