His precision has gotten better overtime although he still needs to focus and has always been stationary while doing so.
The successful targets have always been an inanimate object so obviously a living being would be moving at all times and would obviously be harder to pin point.
The targets mass plays a role,the larger the mass the more chakra and time is needed and kakashi can only use it a limited amount of times as is.
Lastly,Kakashi can use it on himself to transport between the dimensions.
One could create resengans(Naruto),Amaterasu(Sasuke) to force him to use it up as quick as possible.
Others could use there speed to avoid it entirely (Might Guy,A,Rock Lee)
Or others can Like TenTen could send a barrage of weapons to clog it up or block his view of her forcing him to suck up the weapons instead.
Shino could do the same with his bugs.
Choji could use his massive size to create too much weight in his heightened form to make it possible to warp a certain section of him or at least slow down the time the warping is happening thanks to the mass and take the opportunity to strike at kakashi forcing kakashi to stop and attempt to evade him.
