Susano'o causes continuous harm on who's using it because they require constant output and contact with Susano'o for it to remain activated. If the usrer is removed from inside Susano'o, this connection that harms the user but keeps Susano'o active is severed, causing Susano'o to break down and, presumably, the user's pain to stop.

With the exception of EMS, in which case no theres no harm done to the eyes. And to answer the OP's question, like Joker said, its basically a bubble. Once activated, Susanoo surrounds the user protecting them within itself. Usually the user is on foot, which is why it merely appears behind the user, but in the instances where Sasuke is jumping, or when Madara used his PS, the Susanoo wraps itself completely around the user. Think of the Susanoo as a giant hamster ball.

OT: Susano'o is kinda confusing to me. It seems like it's a solid extension of the user, but if that's the case, why is it possible to rip someone out of it so easily?

Since Gaara's sand caught madara from below, i would say that it is hollow, with an empty spot on the bottom.
