Now as we all know that Naruto and Sasuke are reincarnation of Ashura and Indra but in Rtn world Sasuke is just a jerk and Naruto is a high rank jonin so how it all would have ended.I'll try to show through a fanfic below.
In this world Sasuke never left the village because he knew that he can't beat Itachi so he didn't bother to think about revenge.
But one day when he was on konoha street flirting with Sakura he saw Itachi standing near a coffee shop with kisame.Sakura like her usual self was going to punch Sasuke but then she saw sharingan in Sasuke's eyes, she was shocked because as far as she knows him he never uses his sharingan because it is very hard for him to handle and just when Sakura was going to ask him about his sharingan Sasuke makes a Chidori and run towards Itachi.
Itachi looked at him and stopped his hand like he did in original Naruto series but the difference here is that Sasuke is 15 years old.Itachi suddenly kicked Sasuke in the gut (making him fell on the ground) and ask 'What the hell are you trying to do weakling' Sasuke replied 'I don't know I just couldn't control myself after seeing you' Itachi trample Sasuke and said ' What the heck does that mean are you trying to take revenge or something(laughs)' (Unlike in original series Itachi really is evil in this world, he really killed his clan to check his strength and he leave Sasuke because he doesn't consider him worthy of getting killed by his hands but this Itachi is not as strong as original one because he dosen't posses izanagi).
Itachi then start kicking him on his face saying scums like you shouldn't try to act cool and just when Itachi was about to destroy Sasuke's face by continuous kicking a flash appeared and Sasuke vanished from the ground Itachi looked puzzled just then he heard 'What are you doing with Sasuke' from the yellow haired guy standing with Sasuke on his shoulders.Itachi suddenly realised that the yellow haired guy is S rank jonin Menma Namikaze of hidden leaf village he said 'I don't have time to bother with a ninja like him right now' and ran away. Menma on the other hand thought to catch him but Sakura insisted taking Sasuke to the hospital.
After the treatment from Tsunade in hospital Sasuke realised that he's a worthless weakling who can't do anything all of a sudden he got a letter from a bird, he opened the letter and there was a address with a single sentence meet me at my hide out to I'll give you a way of killing Itachi.
Sasuke thinked about it for some minutes and decide to go to that address.Now unlike in real story no one bothered about Sasuke being suddenly disappeared until 3 days of his departure, Menma was the first one who realised it because Sasuke's house was locked for 2 days so he called konoha 11 and told them about Sasuke everyone with not so bother attitude started saying things like it's good if he's out and more but Menma told them that whatever they say Sasuke is still one of them so they should try to find him and go to rescue him.
All of them agreed but tsunade insisted to wait because she saw the emergence of Orochimaru in this case.On the other hand Kabuto was giving Sasuke drugs for speed,stamina and Chakra so that he could handle curse seal that Orochimaru was going to give him Sasuke was feeling good after taking drugs when all of a sudden Orochimaru bite him on the neck and put him in the box like the original story, few moments later he opened the box and Sasuke could feel power like never before
Orochimaru then trained him for 2 years but unlike real story Sasuke in this world missed a lot of the things from the village but he just don't want to go back before killing Itachi.
One day Sasuke was walking in the hall when he heard Orochimaru saying to absorb Sasuke in him to Kabuto.Sasuke was shocked and decide to Kill Orochimaru as he was sick in bed and like real story he killed him but unlike real story Sasuke activates his mangekyu sharingan after killing him because Sasuke really considered him as his master and the pain of killing him result in awakening of mangekyu sharingan.
