I think i've understood how Izanami works, but i'll like to have your opinions about that. 
First of all, this is the main page where Itachi explain the technique:
He says: "the instant you voluntarily decide to act.. yout memorize the bodily sensation of that instant like a photograph, using your eyes". He called this instant A.
The term "body" here is important. That means a body contact with your opponent is needed.
He also add that: "then you replicate the same bodily sensation intentionally and memorize that as well like before". He called this instant A'
That means that to use Izanami this is what you need:
- 2 instants are needed. You must memorize this 2 instants
- The 2 instant you memorize must be the same
- During these instant, a body contact with your opponent is necessary and it must be the same
And like Itachi says, when you've connected these 2 instants, it also memorize automatically the time passed between them and that creates a loop.
So, when did Itachi used Izanami? This is it.
Itachi memorized the first instant(called A) here:
And he memorize the same and 2nd instant here:
And it's only after the second instant that the jutsu is activated, after memorizing the 2nd instant that he called A'. You can notice that his left eye got blind only after this moment not before that.

You can even notice that Kishimoto wanted us to notice his left eye. It's when he memorized that instant.And he memorize the same and 2nd instant here:
