Well, it's possible the eye technique requires direct eye contact, similar to Tsukuyomi. It's also possible that Shinobi can "sense" when the attack is going to occur (Nagato was able to sense when Amaterasu was coming prior to Itachi using it), so that could give an opening to avoid the attack. When all else fails, I bet that certain Shinobi might be able to overcome the KA (perhaps a skilled Uchiha or a Jinchuuriki who's made friends with his Bijuu). I know Itachi planned to use KA on Sasuke, but there's no guarantee it'd actually work.

Look at it this way; Shisui attempted to use KA to prevent the Uchiha Massacre, and not only did he fail, but he was defeated and had his eye taken. So obviously it's not a free win... he even most likely had two chances at it, considering both his eyes are capable of using KA.

The rest of Danzou is pretty lackluster. Even with Izanagi spam, his own jutsu are pretty worthless... he just stands around making you wait 10 minutes to defeat him. A lot of upper S-class ninjas (everyone on the top 15-20 list) wouldn't have much of a problem. He honestly didn't even push Sasuke to the limit, it's just that Sasuke was still unlocking his powers and wasted a lot of chakra figuring out Danzou's abilities.

It does not require eye contact since Danzo used it on Mifune despite his eye being wrapped with bandages.

Sensing it is possible but Danzo can always use Izanagi to survive and then cast KA.

Itachi knows alot about KA's abilities, same goes with Danzo who was planning to use it on Obito who he thought was Madara who had an EMS. Not to mention the opponent wouldn't know if they are being controlled by KA. As for the Bijuu disrupting the chakra of his Jinchuriki, it wouldn't work since Itachi got his chakra disrupted twice when he fought Kabuto, the first time was the sound genjutsu and the second was Sasuke's genjutsu but KA was still working.

Shisui was planning to use it but didn't since Danzo took his eye, he then decided to give his other eye to Itachi.

Kisame can counter KA; Samehada can tell if he is in a genjutsu and can dispel it.

It won't take time for Samehada to notice Kisame is in a genjutsu. Samehada always is absorbing chakra from Kisame. He can't instantly kill himself so, try again.

Once every ten years he can use it. And even then, its not a perfect use. THe one time we saw Danzo use it, Ao calledhim out. All they need is a sensory ninja that Danzo dismisses (and his arrogance lets him dismiss quite a few). Its just not a reliable move.

Expected it to be Yamato the Beast not you Immortal Itachi.. Well it's originally Shisui's Eye, so Shisui > Danzo. And Itachi is an Uchiha and can better handle it so Itachi > Danzo. Also Izanagi > KA.. I think, I have no idea.
