He did not seem to be in too much pain, if you look at his flashback from right after the battle.

he had a huge scar on his chest so im guessing he was wounded pretty badly.

Yeah he wasn't that bad considering his own Weapon was Strike through his Chest.....yeah pretty sure he was what kids these days say "Aiight"

Izanagi! He may have not been injured to near death. Perhaps he knew he couldn't win and bailed!
He would use izanagi secretly at the last second before dying (meaning he was near death). It would not be strategic for Madara to simply run away again without convincing his foe that he was dead, meaning he would have to have acquire serious wounds. Why? It would be more to his advantage that everyone though he was dead, especially in Konoha so that they wouldn't pursue him. Proof of the fact that he faked his own death is that everyone thinks that he died during his battle against Hashirama while he secretly did his plans.

Not near death but a tight inescapable spot. Wounds do not mean the end of a match. Just one hit to a vital spot is enough to convince someone that someone has died.

These mean near death... WTF. When you're stabbed near the heart, I think it's the end unless you have a full restore. O_O (Pokemon joke, he he he he he... no? k.)
