I don't even recall him using limbo at the time(but maybe he was), but why would we expect his limbo to be paying attention to his back when his opponents were in front of him, and he had no reason to suspect Zetsu would hurt him?

Black Zetsu is practically undetectable. We saw him sneak up on Obito to force Rinne Tensei (granted he's not a sensor). He snuck up on Minato (who's actually a good sensor) and nabbed half of the Kyuubi. Sneaking up in Madara isn't so far-fetched. Especially since he thought BZ was apart of him.

I think Black Zetsu mentioned at one point that he was like the earth itself.
