How do the chunnin exams work?

So who becomes a Chunnin? Just the winner of the tournemant? Or does everyone who participates in the primaries become a Chunnin?

The one that demonstrates leadership quality, strategic intelligence, how to use analytical skills etc and ensure their team survives and can complete the mission. In otherwords they'd be able to lead a team hence Shikamaru became a Chunin. Sarada and Shikidai would fit the bill based on how they did things in the chunin exam.

Participants are(were) put through a written, then “survival” test. Those who made it went into the actual one on one fights. Keep in mind you don’t need to win to be promoted, it depends on your performance and the choices made during the test

0 exams are needed, it's moreso the skillset and abilities or a demonstration of applied abilities. In essence a person's competence level and intelligence is tested.

The 4 that shouldn't have become chunin were Hinata, Rock Lee, Chouji and Kiba they don't have any of the skills necessary to be chunin. They were likely promoted for the heck of it or for some other reasons. In Lee's case it's sort of understandable he's an alternate candidate and has some exemptions or someone thing.
