He probably didn't expect him to see him alive. Naruto never met him, he only saw a picture resemblance carved into a mountain. Naruto called him old man and then proceeded to realize their was some importance when he got a better look at him. He is like his mother, shoot first ask questions later.

He knew he was the second. It's just Naruto being himself by calling others with his weird naming. Tobirama wanted Naruto to call him "Nidaime-sama" but you know Naruto's never gonna do that (like the way he calls Tsunade, Jiraiya, and Onoki).
