How did Rin die?

Just a simple question, been wondering for a time, I've seen in the Naruto Inheritors of the Will of Fire movie for example movie that Kakashi stood on the destroyed bridge after his first mission as Jounin with Obito, Rin and Minato, Rin was alive when Obito died but then when Kakashi supposedly ended the mission and stood at the bridge she was gone?

Did I miss something? I didn't read the manga but it feels like they should've included something as Rin's death in the anime..
I just feel like there's a hole in the story somewhere, Kakashi also apologies to Obito and Rin for not being able to protect them at some point(s) in Shippuden..

I guess my general question here is,
How did Rin die?
