Madara and Izuna developed they're MS because they were both very skilled, they trainned and to protect his clan, Izuna gave his eyes to Madara so he could protect them all (the clan) or when Madara got blind, when they made the operation and transfered the eyes of Izuna to Madara, he achieved EMS

Its has been said in the manga and episodes. His brother and himself was rivals and they both got the MS But Madara used his too much and became blind. Itachi said that he took his brothers eyes but Madara said he willingly gave them too him. And out of that EMS. Sasuke should have it too.
