How did she fail to achieve peace?

She was too strong, no one could have opposed her. People even feared her. If she commanded peace, everyone would obey.

Many would say that the Juubi went on a rampage and that was why she lost/died. But she was the most powerful, so she could'nt have lost to the Juubi. Furthermore, she couldn't have lost to her sons either.

So what really happened?

I think, after the remaining chakra of the Shinju had turned into the Jūbi it looked for its chakra. Jūbis was weaker than Kaguya, sure, but maybe it could absorb her like Majin Buu did it with Gohan.

I assume she either turned Into the Juubi, lost control and died and Juubi was released, or the Juubi absorbed her ^

Peace is not someone who enslaves everyone's minds and ensues fear. That's tyranny.

We don't know the extent of her powers or what happened. For all we know, Hagoromo and his brother may have two manned her like Naruto and Sasuke are two manning Madara.

and she probably turned into juubi, since she's now in the tree and the appearance of juubi was not until after she was gone.
