If that happened then hundreds of uchiha would have awoken the MS. in a battle if one uchiha's blood took an arrow for another hten that uchiha would have awoken the MS, or if one brother gave another brother the last bit of food and died then the survivor would've awoken the MS.

Loss of the loved one.... Shishui was Itachi's best friend his loss gave itachi enough emotions to unlock MS

The mangekyo Sharingan is activated by actually seeing someone you hold extremely dear to your heart die. It doesn't have to your brother or best friend. Rin died in front of Obito's eyes, the love of his life. This activated his MS. Itachi died in front of Sasuke's eyes, the activation was delayed until he realized the traumatization (The implant of amaterasu into Sasuke's eyes may have also helped). I would suppose that Shisui saw his father die and got his Mangekyo from that. For Izuna and Madara, I would guess that they saw their best friends, girlfriends, or father die before their eyes and got their Mangekyo from that. So Itachi saw Shisui die in front of him, completely eyeless in a gesture of gratitude.
