IMO this would be a very close battle. Lets break it down.
Tsunade:
Pros: Massive Strength, agil, best medical ninjutsu in the series, highly versatile in poisons and several defensive ninjutsu. Close range fighter
Cons: So far Lacks Offensive jutsu, heavily relies on tie jutsu, more effective as a support, when on offensive mostly will be as a melee shinobi. Weak if separate the distance from her fists.
Mizukage Mei Terumi:
Pros: Has 2 Kekki Genkai, analytical notices the battlefield i.e when she fought sasuke and noticed the hole in the wall and utilized it to corner sasuke with her Lava Kekki, Range fighter
Cons: So far havn't seen to possess a high strength in tai jutsu as seen when she hasn't made the decision to fight Hand to Hand with Sasuke, Weak if you close the distance.

My decision as for a 1v1 i believe Misukage has the upper hand with her more dynamic offensive jutsu. However i will respect Tsunades agil, str, and healing capabilities, but those can be impaired with Mizukage's AoE (Area of Effect) kekki GenKai Boil Release ninjutsu and Lava Release ninjutsu to create obstacles for Tsunade's CQC (Close Quarters Combat).

That it is my explanation on who but please elaborate more onto it preferably against mine so as i may know more of Tsunade's adaptability to this situation.

Well this is vauge, for one there is a lack of important and crucial factors.

Location?: Are the in a desert, a grassy plain, a mud pitt....lets go with mud pitt, just out of random

Gear: what are they wearing in this fight, the stress from the clothes could hinder them so lets say topless so that their upper bodies can breath and lets go with a bikini bottom so they have the highest level of flexibility...so they don't pull anything you know

Now Mei has a level of manipulation on her side which could work, however will it work on the same ***?

Tsunade has a much stronger sheild in the front and rear to cover her but approaching her from the front and attacking the shield head on might cause it to surpress and maybe even cause Tsunade to squeal, I mean...yeah squeal why not.

Now the mud is going to make it all slippry and dirty and hard for either one to graps the other close range , which is Tsunade's speality. Mei has some boiling hot steam and blistering lava to mix with the mud to give her the advantage...

This is a difficult decision hmmm I need to think some more.

Most of you only vote for Tsunade cause HUGE TITS! Of course they're huge, but that's a bad thing. She is all hand-hand combat, you never know when one of those saggy things are going to slide over and get in the way of her punch. I say Mei wins all of those, you can say I'm just a fanboy, but when you look at the facts, it's obvious. If they were to fight, tsunade would get her A** beat. All the mizukage would have to do is simply use the mist tech. and if tsunade got close, she'd melt. It's continuous, so healing would be pointless, she'd run out of chakra, and without that, she has no strength. I also don't think Tsunade could do anything against a blob of lava. Not to mention, she has 3 other chakra releases.
