Go back and reread or rewatch the Sasuke vs Itachi fight. It's obvious that if Itachi didn't have the MS, he would've lost a lot sooner. The MS was used by Itachi to compensate for his inferior abilities.

Sasuke escaped Tsukiyomi with 3T sharingan and reverses some of the effects back on Itachi.
