Healthy Itachi > EMS Sasuke

If you look at it, Amaterasu / enton from both sides are useless, and Sasuke can probably break Itachi's genjutsu. It will come down to a susanoo vs susanoo fight and I think Itachi will win this one because he is legendary weapons and Sauce does not.

Well really we don't know if Sasuke can break Tsukiyomi. Itachi was not trying, and took it easy. If Itachi ever went for blood he will normally get it. Very few characters can defeat, or even injure Itachi. Anyways Itachi for sure.

Itachi wins mid-high difficulty. His susanoo is equiped with legendary weapons, Itachi is smarter than Sasuke and more experienced as a shinobi as well. Itachi also has better speed, taijutsu and genjutsu than Sasuke. Make this EMS Sasuke or make Itachi unhealthy to make this a close fight.

Sasuke tends to spam while Itachi is more conservative of his usage of chakra so IMO, Itachi will end up outlasting Sasuke especially since he is healthy. Itachi has better chakra control than Sasuke as well since mastery of genjutsu requires efficient chakra control. Also he won't have to worry about vision and its been proven in the Kabuto fight that Itachi's reaction skills surpass that of Sasuke.
