We don't know for sure. Many theories suggest he is using Slug Sage Mode while others suggest he is using "Mokuton" Sage Mode. Nobody, but Masashi knows for sure. We can also presume his Sage Mode is "Perfect" as wel..

It's not Mokuton SM,definetly not.The fans diden't understand the scan and assumed it was Mokuton SM.Read my thread it will explain everything.

Is the same as the Buddhist symbol for the sun:

It is known as the 'bindu' representing the sun and a sun god.

Kishi likes to use a lot of religious symbolism in the series so this could be very likely. It would also add irony to the whole situation as Hashirama gets his power from the sun, whilst Madara's objective lies in the moon which is of course Tsuki no me. Sun and the moon, the yin and yang, Hashirama and Madara.

It was said that he got his sage mode from the "humid bone forest" but we don't know what animal it is. Possibly slug? but it doesn't look like a slug
