We saw in Naruto manga chapter 608 last page Guy using Hirudora on Madara.According to him,after using this jutsu his body will become numb and won't be able to move anymore.If he survives it will be due to plot.

Thats the only reason Kishi has kep him alive IMO for him to use 8 Gates against Madara or Obito.
