kamui works like a vacuum or blackhole the problem is not sending it thru kamui (the problem is how much chakra it requires)
kisame saw it coming from a mile away but didn't move because he was too cocky with his absorption jutsu but kakashi on the other hand can predict movement and can be quick to avoid it xd

3- Doton hiding mole
4- Kamui the blast and yes he can. He warped Deidara's Bomb which is roughly the same size. Both are composed of air pressure, they are not heavy target, so they would be swallowed in the black hole of kamui
5- Kamui himself in the other dimension

Actually,every time Hirudora was used we never show the ground getting any damage.So it's a very easy and safe way for Kakashi to avoid Guy's technique.Not to mention that Kakashi can move freely underground with that technique, so even if there is any damage to the ground Kakashi can easily avoid that too,by just moving.

When Guy uses Hirudora, Kakashi wouldn't be able to see Guy, how would he Kamui something he can't see?

Hirudora is transparent. Kakashi would be able to see Guy. Also, he can kamui a certain portion of air if he chooses so he can kamui pressurized air. I also believe he can kamui something as large as hirudora without assistance since he was about to kamui the mazo's head.

It is believable that Kakashi would be adept enough to a Kamui something of that size, which really isn't that much bigger than the blast.
