What happened with Cpt. Yamatato. Did he end up dead or is he alive but the Base just forgot about him.

Also what about the flowerish green zetsu that had that carnivorous head protection or whatever.

Yamato is alive, he was controlled by spiral Zetsu then got trapped in the Infinite Tsukuyomi :
