i think sasuke is pulling off one of his signiture genjutsu where he effect somthing so small an un noticable thats the enemy completely misses it.

or a zetsu has been put under genjutsu and made to look like sauce

One of his 'Signature Genjutsu'? Genjutsu has always been among Sasuke's weakest points, I doubt he'd try to bring it out against someone like Madara, and I doubt someone like Madara would ever fall for the Genjutsu.

I don't think Sasuke will die but I do believe he was really stabbed. I mean this is Madara we're talking about, no one is on his level. Anyone who believes that Sasuke can use a Genjutsu on him or trick him with a clone is delusional. He stated himself that he was the only one who could see through Hashirama's wood clones and he knew that Naruto was a shadow clone when he first arrived on the battlefield with Obito. He isn't that easy to fool.
