Prediction:
Many people (myself included) have been going at this the wrong way. General assumptions for Gear Fourth would be that it covers Luffy's defense or stamina because Gear Second covers speed and Gear Third covers power. This is not the case though considering each of them are simply meant to power him up. Other assumptions involve combine the second and third gear or just an enhancement of the Gear Third.
Gear Second - Blood
Gear Third - Bone
Gear Fourth - Flesh/Muscle
In this case, Luffy has full control of his elasticity and either hardens or simply layers it on himself. This makes his appearance the same but with a thicker build and maybe a darker skin tone due to the build up of rubber. In this form, Luffy gets an increase in strength and is still able to stretch but not as much as he previously could.

Withstand & Redirect Impacts
- If Luffy were to be hit by a punch (even ones imbued with Haki) the impact would simply vibrate within his body and, similar to how Luffy is able to move the air within his bones during Gear Third, Luffy would move the vibrations throughout his body until it reaches his fist the moment his punch hits someone else.
- Luffy can take in multiple impacts to buildup a larger force within his body, resulting in a stronger attack when release
- Luffy can punch his arms together in his usual pose to build up forces on his own like in this picture

Impact Overload
- Luffy cannot hold the impact force in his body forever, so he has a time limit before he must release it somehow. If not released, the entire force would propagate throughout his entire body. Though this wouldn't hurt (because he is a rubber man) it would make him vulnerable in the face of an enemy
- Luffy has a limit to how much force he can withstand. Simply put, a strong enough attack can still hurt him if it exceeds the amount of force Luffy can withstand. In this case, Luffy would still be able to limit the damage he would have received
