I'd say Gaara takes this mid diff. Or high. I mean, with Tsunade's strength there is no way Gaara can hold her down. Also, Tsunade is way faster than Gaara, and has better stamina feats than Gaara.
Katsuyu really isn't useful against Gaara at all, since she's a huge target and can be easily wrapped in sand.

Indoors/places with no way of getting more sand Tsunade takes Gaara Mid diff.
