Ya this is a good point. Basically they all do have last names, but Kishi is not bothering to tell us.

The most important thing you can gather from that is basically the other villages have no real important clans or groups within their village. The leaf is so diverse (obviously Kishi played favorites) that telling all the last names was important. But the other villages don't really matter, thus he hasn't introduced many last names.
