1. You'll more than likely unlock it after your completion of that section are story mode.

2. Is there an empty slot next to the Madara's you've already unlocked? If yes, then shirtless Madara will more than likely be unlocked there. If no, then shirtless Madara will more than likely be available only as an outfit change.
