I think it would be fitting if he looked kinda old (like at least +10-20 years aged), and can no longer open gates.

He learns the wisdom that comes through age and becomes an even more perfect teacher for Lee.

Maybe since his taijutsu has weakened he can focus more on a special personal spin on ninjutsu.

if naruto keeps his gift from the sage..... i see no reason why he can't completely fix him if he was able to make kakashi a new eye..... year nobody dies in this manga but it is suppose to be a happy go lucky ending i suppose.
