Itachi is inside of tsukiyome when he casts it. Madara was also in his genjutsu when he talked to Obito inside it. It's not like he's just gonna chill while everyone else is just standing there

Well lnside Tsukuyomi a few seconds are like days, maybe Madz wants them to live a full life inside the genjutsu and die in the real world after a couple of days with no food and water

That's just crazy! Madara or Obito should allow everyone to have a 2 hour break period or something from genjutsu so they can do basic human needs and sustain their life and then return back to the genjutsu world.
