First to meet the sage?

the way the sage spoke, he had at least met a few others. it doesn't seem like he met hashirama or madara though.

Why would they need to meet him?

I think it would make sense for Hashirama to have, with his Sage Mode seemingly being superior to Naruto's (without BM) and DSM Kabuto even though they both perfected it. Also, his Shin Susenju was what allowed him to defeat Madara w/Kurama, which does not display the supposed equivalency of Ashura and Indra.

My theory is that the Sage was still ignoring Indra's chakra at the time, and that he may have given Hashirama this technique, or just improved his Sage Mode in general. And/or the Sage taught him things and generally made him more knowledgeable about chakra since Hashirama was renowned for his prowess in sealing and dealing with biju.

Its possible, but I feel especially in the chapter Sasuke asked Hashirama about the villages and cycle of hatred he would have told him he met the sage and chose a different path and perhaps he was responsible to a degree for what was going on, I definently don't think madara met him, he was exceptionally power hungry even for an Indra reincarnation and he as they said surpassed the transmigration and grew closer to the sage and Kaguya, so I don't think either met mainly because I feel Hasi would have told someone probably Sasuke, or have learned some other way to handle Madara, but Madara I would say for sure didn't meet him.

No they haven't the Souls were reborn for a time in Madara and the First Hokage in the SO6P Sons but they never completed the circle to be reborn the way Naruto and Sasuke have, they never address the problems.
