is the 'fate' which the victim has to accept determined by the izanami caster ? or is there a predetermined/innate fate for everyone based on who they are and what they do?

Condition =/= fate.Most people don't even realize how Izanami works, the user can not decide the condition (fate) but he can chose (more like forced to chose) the starting point of the loop.
Basically, the Izanami user has to chose a moment A1 and then try to replicate as many sensations as possible the user and victim underwent during that moment (sword stab, sensation of stabbing, sensation of getting stabbed, seeing the crow clone, re-seeing the clone crow, getting horn cut, getting a water droplet to touch you etc, the more sensations the better) and thus create the moment A2, then connects the moment A1 and A2 to create the loop.
Izanami is practically unusable in a battle against high tier opponents.

Condition = accepting yourself = break the loop.
It has never been stated or hinted anywhere that the user holds any kind of control over which rule Izanami will decide. Which has always been accepting th results of your actions aka acknowledging your past mistakes and accepting your real self.
