There are several naruto realism artists, but the ones who do the most and are my favorites are narutopants, olggah, cheo36 and zetsuai89.

Narutopants mostly does realism for manga scans. olggah does character portraits. cheo36 does battle scenes and zetsuai89 does all manner of concepts, though he's a bit homoerotic in his works and comes off as obsessed with Uchiha characters lols.
