That was explained when he was 13. Again, you are basing all of this on assuming that he didn't improve at all since he was 13. Minato only said that because there were multiple enemies that Kakashi ran at. If he's going 1v1 against Tsunade it doesn't matter if he has tunnel vision since shes the only one he needs to look at.
