EMS is simply 2 blood relatives eyes merging to create an eye that never fades. Simples.

When Madara was revived with Edo Tensei he got his eyes, even though they were transplanted into Nagato.

When Nagato was revived he had his eyes, even though Tobi had at least one in his right eye socket.

Edo Tensei simply revives the target in their prime, or the closest it can. Itachi, Madara and Nagato all getting their eyes in the form they had in their prime, simples.

Masashi Kishimoto created the Narutoverse, then sent down the Uchiha to rule it.

Danzo saw this threat and pressured ANBU to get Itachi to kill them, since no others could.

I'm of the opinion that both brothers could theoretically get the eyes. I believe that the eye surgeries of the manga consist of simply taking the eye out of one host and implanting it into the socket of another, similar to Kakashi receiving Obito's eye and Ao receiving the hyuuga byakugan.

However, coincidentally in both cases of a brother taking the others eye, one of the brothers was always dead (Izuna and Itachi.) Itachi did claim that unlocking the the mangekyou in general was not something everyone in the clan was capable of doing, and the odds of both siblings in a family unlocking the eyes must have been equally rare. Perhaps Shisui had a brother that died before being able to unlock the eyes?

If Itachi was not dying/ hoping to redeem himself through death, it seems logical to me that both he and Sasuke could have received eternal mangekyou sharingan. However it did take Itachi's death for Sasuke to unlock his mangekyou in the first place, so we will never know.

What's there to explain? Two MS pairs fuse together to form a single EMS pair. Ergo, who donates the MS pair to the recipient would be left without eyes. Which is why Sasuke is the only EMS holder after Madara.

You take someone's MS and literally force it into your eye socket, with your eye in (note: your MS HAS to be active or this won't work). If done correctly, it'll push your eye backwards into your skull, and their eye will be in the front. You'll see out their eye, and with yours touching their's from inside your skull, it activates the EMS.
