Actually there is pretty good explanation which I can think of:
Madara's body evolution: Prime Uchiha body -> Prime Uchiha with Senjuu DNA body -> Old Uchiha with Senjuu DNA body with Rinnegan.
Looking at what the Rinne Tensei has done and as it brought Madara in young body with Senjuu DNA... I am sure that he was revived in second state in Madara's body evolution.
However How Kabuto was able to do that... stays mystery for now. Probably injected more and more Senjuu DNA in Madara along with his mastery over medical Ninjutsu was able to revert Madara's old body with Senjuu DNA to Madara in young body with Senjuu DNA. It allowed him to still keep his Senjuu and Uchiha DNA combined in one body after the Rinne tensei was used on him. Now The mystery itself is Rinnegan, even if Madara's body was reverted to young/prime but with Senjuu DNA... then he should NOT HAVE Rinnegan, unless once awake Rinnegan can never go away... as it may make pernament imprint on DNA and Edo tensei uses DNA of person to work
Now The plot hole could be connected with Edo Tensei, to be more specific the way Edo Tensei bring revived one in body. However Edo Tensei was very inconsistent, so maybe user can choose in what state of body revived one will be revived. Now Everyhting Rinne tensei has done is still keeping his young and prime body with Senjuu DNA but loosing fake Rinnegan created by Edo tensei. It could be due to Rinnegan still exisiting in this world... and it can not duplicate or it simply was taken out of his body beforehand... so Rinnegan simply returned to last state of body was as Edo tensei bring it... so Rinnegan was recreated in Young Prime body with Senjuu DNA.
In short/ In Order :
1) Madara transplanted his double Rinnegans in Nagato's eyes
2) Madara dies
3) Kabuto found Madara's old body with Senjuu DNA intact and inject more Senjuu DNA while using medical ninjutsu on it
4) Kabuto reverts Madara's old body with Senjuu DNA intact to his young prime body with Senjuu DNA(Just after Madara inject Hashirama's DNA in himself while being in prime age.)
5) Kabuto used Edo Tensei to brought Madara's soul in sacrificed living body which being to take shape of Madara's soul
6) The last state of Madara's body was Young Prime body with Senjuu DNA... but Edo Tensei bring complete body so The Rinnegan is forefully recreated from saves on DNA.
7) Rinne Tensei was used on Madara's soul to get revived... but Madara's soul was already trapped in Edo Tensei's ashes/dust body so Rinne Tensei just canceled Edo Tensei effects on Madara's body and brought it to life.
8) Canceling Edo Tensei effects removed Rinnegan from his eyes and turned it to dust while preserving Young and Prime body due to alteration/modification made on his REAL body BEFORE BEING BROUGHT BY EDO TENSEI.
Summary: Prime and Young body was NOT MADE DUE to EDO TENSEI, while Rinnegan appeared in his eyes due to being created BY EDO
it can be even more simple but it bring another plothole.
I mean Rinne Tensei bring soul back to physical body/remains and ressurect you. However Madara's soul was trapped inside edo tensei body so Rinne tensei used fake body to turns it into real while Madara soul was there. Rinnegan faded away due to existence of same eyes doubled is impossible. If rinnegan was destroyed then edo tensei fake rinnegan could turns into real.
But in this case Madara's revival would be plothole as Edo tensei was not excepted to be used and Kabuto's interference was random from nowhere. That only mean that my first theory is right or Madara could be brought in old body without eyes which does not make sense for Madara wanting it...
My second theory in order:
1) Kabuto reverted Madara's body to young and prime with Senjuu Dna
2) Kabuto then took sample of dna from this young madara body ( edo tensei requires sample of dna perspn you wish to bring back, then soul of revived perspn will be pkaced in fake body which turns into body from which sample of dna was taken. Young body of Madara in this case.
3) Kabuto did ritual and brought Madara's soul back into sacrificed living body which took shape of young body from which Kabuto took sample of DNA
4) Madara get brought into young body with Senjuu Dna with Rinnegan either created by imprint of Nagato's Dna or Madara's Dna has save about Rinnegan being awoken.
5) Rinne tensei get used and Madara soul being trapped in edo body forced Rinne tensei to ressurect moving corpse turning into Living body.
*Rinnegan disappeared either due to Nagato's Dna being used and it was not original part of Madara's body or Rinne tensei couldnt ressurect his eyes also because Madara eyes still existed in this plane/word. It could not br duplicate either way.
Please keep in mind that we still did not get full explanation how rinne tensei work. Only parts so it is not plot hoke and Kishi can explain it surprising all Naruto fans.
