Itachi has the two most powerful known genjutsu's in Naruto. Madara gets caught in either one and loses if he gets close. If he keeps his distance, Yata mirror is in play. Counters Limbo or any other tech he has. Totsuka blade is also used if he gets close.

Rinnegan blocked Infinite Tsukuyomi. I have no idea what you expect Kotoamatsukami to do here.

IT works differently. The light needs to touch you in order for it to take affect. Sasuke's rinnegan only mangaed to block IT because he had the Sage's Sakura. He had Rikudo Sennin chakra. That's why his rinne sharingan was able to withstand the IT genjutsu.

This Madara we're talking about doesn't have Rikudo Senin Chakra (read the thread tittle). He cannot block IT if it was casted on him. Meaning he cannot block KA. KA instantly takes affect and finishes him off.
