Think of it as a sort of Matrix wherein you create a virtual world.

The user can control virtually every aspect of the perception of the victim, including time, laws of physics and other stuff.
It is very taxing for the user, too, however.

Eye of the Moon plan is Madara's and Obito's plan of world domination by casting a neverending Tsukyomi on the whole world - this would allow them to control everyone.

They know, but itachi is able to manipulate time so much that he can make 3 days out of 1 second. They can't break the jutsu because they're never given enough real world time to effectively use any form of genjutsu release (pain, rotating chakra, reversal, etc.) That and the fact that he can manipulate everything in it from mass, to location makes it even harder to get out and easier for him to kill you if he sees fit.
