listen well focusing on your chakra wont help, Yin/Yang is not some kinda of chakra, its an attribute of your chakra. What you'll need to do is combine the yin half of water with the yin half of fire, and yang half of fire with the yang half of water, the result will depend on how well you balance the 2, if in the end either one of the yin/yang's energy is more then the other, it might not be as effective, it might not even have a effect(water would cancel out the fire) or it might back fire, so it would be advisable to complete balance the 2 elements individually 1st so the amount of yin or yang energy dont exceed that of the other elements energies.
It would require a lot of focus and you might not even be able to control the energies but keep trying

I continue meditating and focusing my water and fire chakra's in either hand until I've balanced the yin and yang in the water, and the yin and yang in fire. I then stand up putting my hands together and I begin to combine the two elements keeping and equal balance of yin and yang in both the water and fire.*
