If the Sharingan is based on the eyes reflection of chakra in the brain that reflects the optic nerve.

How can you access it's abilities through simply having the eye? Wouldn't it also require brain and nerve surgery?

Same can be said for the Byakugan and Rinnegan as they may or may not be direct mutations of one another.

It seems doujutsu by themselves are attached to a castor's astral figure like Obito transfering his eye to Kakashi in astral figure so it doesnt apply much in the physical context.

Madara cutting Kakashi's eye and replacing it into his socket without nerve attachment was funny.

makes no sense.
but if the brain activates the eye, then maybe when transplanted, the eyes lingering chakra unlocks the brains capacity to use it?

Obito didn't transfer his eyes, he transferred his chakra, and that turned Kakashi's own eyes into sharingans.

it's true that the whole concept of sharingan, I presume byakugan as well, is that the chakra gets pooled into the brain that it affects the user's eye.

If you think about the whole "doujutsu" transfer, you can tell that the chakra is obviously stuck in the eye itself now. Much like how bijuus residing on someone's body.

If you take a look at Kakashi's or Danzo's sharingans, they are permanently active. This means that the chakra never recede and gets pooled into the eye.

"How come it never deactivates and drain the person of chakra or that it never goes blind?" Well, at this point, it just now acts as a catalyst. The original chakra never gets used but the person's own chakra. That is why it doesn't run out of chakra despite it being active and just being covered.
