It's just a transformation jutsu that feeds off the chakra in her forehead seal, so it's permanent as long as she doesn't run out of chakra in her forehead.

It's still a normal transformation, it just doesn't use the chakra she normally uses, which is why she reverts back to her real form after she uses her forehead seal.

Don't you think she'd just let it wear off when fighting an enemy like Madara? I mean, Sakura did mention that she doesn't NEED to use it to look young.
What if Tsunade is weaker in her old form than in her young form, thus she is forced to keep her transformation jutsu on at all times when fighting an enemy.
