I would say it's most likely a test tube baby.

We are talking about Orochimaru after all, and his obsession with preserving genetics. Maybe he took DNA from Toneri and used his own DNA to grow a child in one of the tubes. Toneri's DNA could have been hard to find a genetic component strong enough to merge with, and Orochimaru's basically the best specimen other than Kabuto, or Hashirama.
