so6p discovered how to use chakra in nature like manipulate elements and etc. but sage mode is getting chakra from nature but in a way naruto can use it to control other elements with his extended reach chakra xd

"sage" is just a title normally used to describe a master of a certain skill. Using this, the "So6P" was a master of the six nature paths. While a sage mode ninja is a master of combining their chakra with nature chakra into a form representing the animals who taught it to them (ie toad sage, dragon sage, slug sage?)
