That he had the Rinnegan? Nagato was still young when Madara implanted it in him, so didn't they ever see his eyes and think that it was weird and also his hair couldn't have been covering it for all this time they must have seen it a least once :erm:

his parents were most likely aready dead.. and we dont know exaclty how young nagato was when the eyes were transplanted and we dont know if nagato even knew they were transplanted so its possible that it was done whiles he was in a genjutsu or watever

I think that maybe Madara put them under genjutsu "not to care about son's eyes".

They must have noticed. And we know that Nagato got his eyes before his parents were killed by Konoha, so option with genjutsu seems plausible.

Not likely that we will know eventually by a manga scan. Nagato has now almost no possibility to appear in manga again.

I have two options: i believe that he had the rinnegan all along but his parents didn't really care or mind if he had those because they thought of them as normal eyes. ( like blue, brown, and black eyes.)
The second option is that he had madara's eternal mangekyou sharingan eyes.( they were black because he didn't learn how to activate them.) When his parents were killed, his eyes awoke into rinnegan( remember that he has senju DNA and that he has the power of the uchiha which is the sharingan, so he can awaken the rinnegan.) and that is when he first showed his rinnegan in the manga and the anime.

i believe he can't switch back to sharingan because he is not a pure-blooded uchiha just like kakashi who can't deactivate his sharingan.
